Questions & answers
If this is a medical emergency, call 911. Everything below assumes you have a moment to read.
No appointment and no referral, ever.
Board-certified, every visit.
Answers during the visit, not after it.
Being wrong about it is not a wasted trip.
If you think it might be an emergency, come in. You are not expected to diagnose yourself, and coming in to find out you are fine is not a mistake — that is what an emergency room is for. Coverage for emergency care turns on whether a reasonable person would have thought it was an emergency, not on how it turned out. (The detail, with sources, is on Billing & Insurance.)
Chest pain, trouble breathing, stroke signs (sudden weakness, drooping on one side, confusion, trouble speaking), severe pain, a bad fall, a high fever in an infant, uncontrolled bleeding: those are emergencies. If you are unsure, call us on (480) 237-4922 and describe it. We cannot diagnose over the phone, but we can usually tell you whether what you are describing belongs here.
If someone cannot travel safely or is getting worse quickly, call 911 instead of driving. An ambulance starts treatment on the way.
No — and there is nothing to book. No appointment, no referral, ever. Walk in.
Yes. 24 hours a day, 7 days a week, 365 days a year, including weekends and holidays. CT, X-ray and a full laboratory are in this building, which is the part that actually matters at 3am — an ER that has to send you somewhere else to be scanned is not open in the way you need.
12000 N El Mirage Rd, El Mirage, AZ 85335.
This is the building you are looking for — EMERGENCY 24/7 on the parapet, 12000 on the wall, and the red canopy is the walk-in entrance.
Photo ID, your insurance card, and a list of your current medications if you have one. Come anyway if you have none of it. Nobody is turned away for arriving empty-handed — paperwork can catch up while you wait, and it never decides whether you are seen.
You are seen by a board-certified physician, every visit — at every hour, with no overnight or weekend arrangement where that stops being true. Around them are nurses, radiology and laboratory staff.
(Not every physician here is boarded in emergency medicine specifically — we explain that plainly on our team page rather than rounding it up.)
Here. CT, X-ray and a full laboratory are in the building — staffed around the clock, not on call — so results come back during your visit rather than in a phone call days later. Ultrasound is called in from a mobile service.
That is the practical difference between an emergency room and an urgent care that suspects something serious.
We are a freestanding emergency room, not a hospital, and this is the answer worth reading before you need it.
We treat and stabilise you here. If you need to be admitted, or need hospital-level inpatient care, we arrange the transfer to a hospital. Most visits do not end that way — people are treated and go home.
Yes, every day. We are not a pediatric emergency room, and we should say so plainly — but El Mirage and the cities around it have a lot of young families, and a large share of who comes through the door is children: fevers, falls, breathing trouble, scorpion stings.
If a child's condition is severe, or you cannot travel safely, call 911.
You are monitored while you are here, and the team is trained to respond immediately if your condition changes. Tell someone the moment you feel worse — do not wait to be asked.
We can hold and monitor you here for up to 24 hours. Not every visit resolves in an hour — sometimes the right answer is to watch you, repeat a test, and see which way things go.
That matters because the alternative, at a facility without it, is being transferred to a hospital simply because nobody can keep you. Being observed here means a decision made with more information, and for many people it means going home rather than being admitted.
If you do need hospital-level inpatient care, we arrange the transfer.
No. We are a freestanding emergency room, and the difference is not a technicality — it changes what we can do for you.
CT, X-ray and a full laboratory are in this building, so what is wrong can usually be established during your visit rather than at a second appointment somewhere else.
A board-certified physician sees every patient, at every hour.
We are open 24 hours a day, 365 days a year, with no appointment and no referral — including the hours when nothing else near you is open.
If you need hospital admission, we treat and stabilise you here and arrange the transfer.
An emergency room visit does bill at emergency rates, and we would rather say so plainly than have you learn it from an envelope. We do not balance bill for emergency care, and HealthPass pays members' out-of-pocket costs — including for visits already behind them.
And if you are unsure whether what you have is an emergency: come in. That is not a question you should have to answer about yourself, particularly at 3am.
Not today. We are an emergency room only, so routine physicals, prescription refills and the ongoing management of a chronic condition are not services we are set up to provide. That is a statement about what is in this building, not about you.
Emergency care is a different matter, and that we do around the clock — a chronic condition that has become an emergency is exactly the kind of thing to bring here, at any hour, without an appointment.
These answers match Billing & Insurance exactly. If one changes, both change.
We bill every insurer. And the honest version, which most freestanding ERs bury: we are out of network with all insurance plans.
For emergency care that does not cost you more. There are federal and Arizona-specific protections covering emergency billing — and separately, our own policy is never to balance bill any patient, which covers everyone those protections might not. (The detail, with the official sources, is on Billing & Insurance — one page, so there is one place to keep current.)
You will never pay more than your in-network deductible and coinsurance here, whoever insures you.
We do not balance bill. Ever. Not for any patient, whatever insurance you carry, and not when your insurer pays us less than we billed.
You will receive two claims — one from the facility and one from the physician who treated you. That is how emergency care is billed everywhere, and it is not being charged twice.
Probably not — an Explanation of Benefits is not a bill. It is a statement of what your insurer paid. EOBs often use severe language about care being out of network; that language is not a demand for payment from us.
Call (480) 339-4825 and read it to us. That is the fastest way to settle it, and it is free.
You are welcome, and the care comes first — everyone who arrives is assessed and receives an emergency medical screening, whatever you can pay. Ask about cost whenever you like; most self-paying patients do, while they wait, and the cash-pay form is filled in while waiting too. Being seen never waits on it.
Cash, cheque and card. If you need financial assistance, help understanding a bill, or a payment plan, that is a conversation the billing team has every day. Call (480) 339-4825.
Yes. If you were hurt at work, your employer's workers' compensation coverage is normally what pays rather than your own health plan. There is a filing deadline, so do not sit on it — the Industrial Commission of Arizona sets out who is covered, the deadline, and how to file.
Yes. Come in.
We are not in Medicare's network, and that does not cost you more. A federal law called EMTALA requires any emergency room to examine and stabilise whoever arrives, and a second one — the No Surprises Act — limits your share of an emergency bill to your ordinary in-network copay, coinsurance and deductible. We do not bill you for the difference.
Bring your Medicare card and any supplement plan card if you have them, and come anyway if you do not. Billing & insurance, in full
Come in — the obligation to examine and stabilise applies whatever your coverage, as does the rule against balance billing your emergency care.
For what your specific plan covers, call our billing team on (480) 339-4825. AHCCCS, VA community care and IHS purchased/referred care each work differently and they change; an answer from the billing team will be current and it will be about you.

About your bill, before you worry about it
If something arrives from your insurance company after your visit, it is almost certainly not a bill. It is an Explanation of Benefits — a statement of what they paid. The large number on it is not what you owe.
For emergency care, federal law limits your share to the ordinary in-network copay, coinsurance and deductible your plan already sets. We do not bill you for the difference, and we never will.
If anything you receive is unclear, call us before you pay it. Our billing team would rather explain a statement than have you pay something you do not owe.
